Health Homes Case Manager - Provides case management services to individuals with qualifying chronic illnesses. Evaluates individuals' social, health, and mental health needs to provide appropriate referrals to community resources and providers. Contacts eligible individuals by mail and phone to explain the Health Homes program. Maintains weekly phone contact with interested individuals. May serve as the initial agency contact to the public and other agencies/providers. Meets with individuals in their home or in the office to assist them with enrollment in Health Homes.
